Natural Health DC LLC’s program commitment outline and Purchase Policy and proceedure.
 
Natural Health DC LLC has available for Purchase (1) program. This program is called “Get Your Life Back”.
 
The “GET YOUR LIFE BACK” Program includes:
 
(2) Information packets sent via email by a  Natural Health DC LLC representative and will have the heading, “Intake Form” and “Participant Agreement Form”.  These packets will be sent to the client’s email no later than 24 hours after Deposit/Payment is processed. It is the responsibility of the client to read and fill out packets and have ready to present to Dr. Antoine Chevalier at the beginning of the first day of session for review. The representative of Natural Health DC LLC will make themself available to answer any question or concern with respect to completing these packets. 
 
(2) 1 day sessions with Dr. Antoine Chevalier. 
 
Each day will normally consist of (6) to (8) hours of work with the client and Dr. Antoine Chevalier. 
 
The exact time spent each day will be decided by Dr. Antoine Chevalier and will vary with each client.
 
The above 4 points encompass all that the 2 Day “Get Your Life Back” program include. 
 
 
Natural Health DC LLC’s Purchase process.
 
Natural Health DC LLC’s purchase process will include the following forms and process:
 
Purchase Forms:
 
(1) Pre Approval Form for each charge made either to a reflect a “Deposit” or “Paid in Full”.
 
(1) Receipt provided by the Natural Health DC LLC office to purchaser via email.
 
(1) Invoice reflecting “Deposit with Balance Due” or “Paid In Full”. This form will identify Date and time of appointment and location that services will be provided. 
 
Purchase Process:
 
Natural Health DC LLC will only start a Purchase Process if the scheduled date for sessions with Dr. Antoine Chevalier is within (2) weeks of the purchase process. Natural Health DC LLC will work with clients to set a appropriate date and time and only within that (2) week timeframe with accept deposit/payment.
 
The purchaser of the “Get Your Life Back” program will be responsible to make verbal contact with payment Card Issuing Company and clear verbally their acceptance of a Charge of deposit/ Payment request about to be done  from Natural Health DC LLC for the agreed upon amount being made prior to attempt of charge by Natural Health DC LLC. 
 
A “Pre Approval  Form” will be sent from the representative of Natural Health DC LLC to the purchaser via email. This form is to be read and Understood completely by the purchaser with any questions or concerns addressed to the satisfaction of the purchaser prior to being filled out, signed then returned to company representative via email. Once returned via email to Natural Health DC LLC or its representative, the office will process via Phone call the “Deposit” or “Full Payment” that is agreed upon by purchaser per the signed pre approval form. By initialing / signing where appropriate on the Pre Approval Form the purchaser acknowledges the purchase, what it includes and that this is a NON REFUNDABLE CHARGE. In addition that Neither Natural Health DC LLC or any of its representative will at any time make any form of promises or guarantee with respect to the outcome of services purchased to any Client at anytime.
 
Once approval of charge is received by the office of Natural Health DC LLC, a emailed copy of the charged receipt will be sent to purchaser. The purchaser will signed and return this receipt back to the Natural Health DC LLC office or its representative via email the same calendar day. By initialing / signing where appropriate on the Charged receipt the purchaser acknowledges the purchase, what it includes and that this is a NON REFUNDABLE CHARGE.  In addition that Neither Natural Health DC LLC or any of its representative will at any time make any form of promises or guarantee with respect to the outcome of services purchased to any Client at anytime.
 
After that signed reciept  is returned to the Natural Health DC LLC office or its representative a Invoice will be sent via email reflecting the charge for Deposit with balance or  Payment in Full and this Invoice will have a space on both pages for initials and signature. By initialing / signing where appropriate on the Invoice the purchaser acknowledges the purchase, what it includes, Date and time of appointment / location and that this is a NON REFUNDABLE CHARGE.  In addition that Neither Natural Health DC LLC or any of its representative will at any time make any form of promises or guarantee with respect to the outcome of services purchased to any Client at anytime.
 
Both day’s of session are provided at the Natural Health DC LLC office located at 1629 K St. N.W. Suite 300 Washington D.C. 20006. It is sole responsibility of the client to be at the office for agreed upon date and time for session work with Dr. Antoine Chevalier. If for any reason or circumstance the client does not participate in both agreed upon scheduled sessions this will not provide reason for a return of funds. As previously stated on all forms this is a non refundable payment. Any other location and time must be agreed upon by both parties on the Invoice and signatures required.
